The nomination form is composed of 3.5 pages to submit along with an additional 3.5 pages about the hall, the nomination criteria, guidelines and the 2021 deadline. Please complete and return the first 4 pages of this nomination form, along with supporting documents and letters of support.
In addition, the required primary photo and the one page summary of the nominated candidate should be included as an attachment.
