Please note that the application deadline for 2021 has been extended to October 15, 2021. due to issues with the ongoing pandemic. all applicants who submitted applications by the original date have been notified. The application date for 2022 is still May 1, 2022.
The nomination form is the MANDATORY minimum for an application, along with a one-page bio/summary and 8 x 190 photos,; it is composed of 3.5 pages to submit along with an additional 3.5 pages about the hall, the nomination criteria, guidelines, and the revised 2021 deadline. Please complete and return the first 4 pages of this nomination form, along with supporting documents and letters of support.
Please ensure it is sent to the proper mailing address: PO box 36075, Halifax, NS B3J 3S9 PLEASE DO NOT SEND it to the 4-PAD Arena Centre as no Board /staff members are present there.
In addition, the required primary photo and the one page summary of the nominated candidate should be included as an attachment.
